Psychiatrist Natick, MA "I was an early adopter of a paperless office. I began writing progress notes for my patients on MicroSoft Word documents back in 1991. I was generally happy with my system until a conversation with a risk management consultation alerted me to the problem that the integrity of my record was difficult to establish since these Word files could be edited after the fact. I saw the need for an electronic medical record and discovered MentalNote 3 while attending a CME program in April 2010. I was attracted to MentalNote because it was written by a psychiatrist and was geared toward psychiatric practice. It fulfilled my wish to continue having a paperless office, while also being able to establish the integrity of my record. My progress notes are actually better now than they were before. Now every note has a mental status exam and a five-axis DSM-IV diagnosis. The fully editable templates allow me to quickly check-off patient's subjective complaints as well as features of their mental status exam, but I also have space to enter free text. I particularly appreciate being able to import the previous note, since that allows me to complete a progress note on some patients even more quickly. Each patient has a complete medication list and a record of when prescriptions were written -- I use the latter many times each day in my practice to determine if it is time to write a new prescription for the patient. In MentalNote, one can print out prescriptions, but I am using a separate e-prescribing program so I don't need that feature. However, if I clicked the "print" button in MentalNote 3, the patient's progress note was automatically updated with a notation that a prescription was printed (including the number of pills and number of refills). I would then edit the progress note text by changing the word "printed" to "e-prescribed" when appropriate, and in this way I was able to keep track of all the times I e-prescribed. That proved to be very important when I realized mid-way through this year that I had neglected to submit the e-prescribing G-code for my Medicare patients. I was able to review my progress notes and establish exactly when I had e-prescribed during a session with a patient. In a very short time, I had all the information I needed to submit all my G-codes."

I am happy with using it while I see my patients in my office and only rarely has a patient objected to my typing while in session. My notes are much more detailed and complete, the check lists are very helpful although I also type out detailed notes quite a bit as well; my prescriptions have less mistakes and are much faster due to printing them from lists on each patient from within the individual patients profile; special letters or correspondence for a patient can often be typed with the patient collaborating with me quickly--never do I have to dictate a letter to my bookkeeper any more; my history taking is more thorough as I can go back and see where I have left out exploring about a patient's past history in followup sessions by going into the history section for each patient; I often refer to the patients medication history without having to go back to previous notes to see what happened with previous trials of medications when trying to decide what to do next with medications when patients are not responding adequately on their current regimes. These are just a few of the advantages of using MentalNote. Also, fast typing is not needed as the check lists can almost be all that is used (if desired) in documenting sessions with patients, except for entering diagnoses and medications."
